Information I Will Collect & Process from You

I will collect personal information about you in order to carry out my professional and contractual obligation to you.

This will include, but is not limited to:

• Your name and contact details (address, telephone number, and email)
• An emergency contact name and contact details
• GP Details/Other care providers details as agreed
• Notes from our sessions/appointments
• Forms completed by you in the course of our relationship together

Use of Information

Your information will be used to manage our relationship together and to send you information about my practices services or information about other services that are relevant to our work.

Your information will never be shared with third parties or sold, with the exception those matters covered in our contract together (e.g. for safeguarding or medical purposes).

Storage of Records

Any records that I keep about you will be held securely in an electronic password protected system.

Any notes written in a session will be transferred to the electronic system and the written notes securely destroyed.

Your records will be stored for 7 years after you leave the practice. After this period, your records will be destroyed.

Your Right to Access

You have the right to view or obtain copies of any information that I hold about you.

This is to be provided within 28 days of your request and in either electronic or paper format.

You have the right to ask for any incorrect information to be amended.

You have the right to ask for any information that I hold about you to be erased. This is not an absolute right and only applies in certain circumstances.
